📋 Intake Questionnaire Scanner
OCR · AI Parse · Confidence Scored

Scan the patient's completed paper intake form and upload it. Unifyr reads it with OCR, extracts all answers, and maps them to the patient's profile. Fields the system is uncertain about are flagged for staff verification before going into the chart. Once verified and applied, the Overview tab updates automatically.

⬡ Unknown EMR Parser
AI-Assisted

Patient from out-of-state with records from an unsupported EMR? Upload any exported file — HL7 v2, C-CDA XML, FHIR JSON, PDF, or plain text. Unifyr parses and normalizes it into a standard profile with per-field confidence scoring.

About EMR Integrations
Unifyr supports SMART on FHIR R4 authentication to securely pull patient records from Epic, Oracle Cerner, and MEDITECH systems.

How it works:
  • You log into your hospital's EMR using your own credentials
  • The EMR issues Unifyr a time-limited access token scoped to your access
  • Unifyr queries patient records on your behalf when you view a chart
  • Tokens refresh automatically — no need to reconnect each day

Security & Compliance:
  • Each connection is scoped to your individual EMR account
  • You can only see patients the EMR shows you (treatment relationship enforced)
  • All access is logged in both Unifyr and the source EMR
  • Disconnecting revokes the stored tokens immediately
